Please provide the school with a copy of the applicant’s most up-to-date immunization record.  Saint Mary’s Hall requires that every student must be vaccinated against vaccine-preventable diseases caused by infectious agents, in accordance with the up-to-date Texas Minimum State Vaccine Requirements for Students Montessori - Form 12. 

Saint Mary’s Hall does not allow exemptions for reasons of conscience.
